A New Era of Care Is Coming to Texarkana
Our state-of-the-art facility will open in Spring/Summer 2027.


A New Chapter of Care for Texarkana Seniors
Bailey Creek Health and Rehab is proud to announce the start of construction on our new 71,800 square-foot skilled nursing facility, located at 5111 Jefferson Ave. This modern replacement building will allow us to continue serving the Texarkana community with an enhanced level of comfort, care, and clinical excellence.
Slated for completion in Spring/Summer 2027, the new facility will feature 138 resident beds, including 88 private rooms with private showers and 25 semi-private rooms designed for comfort and accessibility. Residents will enjoy inviting community spaces such as a large dining room and two interior courtyards, along with a streamlined layout that supports ease of movement and daily living.
To support recovery and rehabilitation, the new campus includes a 2,000-square-foot therapy gym equipped for physical, occupational, and speech therapy, plus an outdoor therapy track that promotes functional mobility in a real-world environment.
We are also expanding our dedication to memory care with a 22-bed Memory Care Neighborhood built to provide safety, structure, and specialized programming for individuals living with Alzheimer’s disease, dementia, and related conditions.
Because this is a replacement facility—not a new business—much of our experienced team will transition with us from the current building. Additional hiring needs will be finalized closer to the opening date.
Bailey Creek will continue offering short-term rehabilitation, long-term care, and specialized memory care, all within a state-of-the-art setting designed to elevate the resident experience. We look forward to welcoming our community into our new home in 2027!
